General Forms and Information
Application Questionnaire
The very first form that is filled out by the veteran for vet court consideration. This can be filled out digitally on the "application" tab.
General ROI
This form is completed so that we may communicate with your treatment providers outside of the VA and VSO's. This is also where veterans add or remove family members they wish to share information with.
Court Agreement
These are the rules and regulations of the program that veterans agree to abide by for the duration of enrollment.
Documents and Forms for Active Participants
Handbook
The handbook given for you to follow for the duration of your time in the program. Please bring it with you to every court appearance.
Travel Request
There is one on page 23 of your handbook, but if you need another you can download it.
Community Service
There is one on page 25 of your handbook, but you may need multiple if you complete your community service hours with multiple organizations.
NA/AA Meeting Log
This is to be completed and submitted when you are required to attend meetings, regardless of which type you are ordered to attend. (Virtual or in-person).
30 Meetings in 30 Days
Complete this form if you are required to complete 30 meetings in 30 days.
Budget Sheet
You may be asked to do this more than once during your time in the court, but there is only one in the handbook.
